Music Express Contract Agreement

            This contract agreement is between________________________and MUSIC  EXPRESS D.J. SERVICE of  the whole state Florida                                                                MUSIC EXPRESS will perform at location, time, and date stated below:

*NOTE: Changing the Location, Time, and Date of the agreement permissible according to the availability of MUSIC EXPRESS’S Schedule,                                                                                                                within a 5 day notice.

    MUSIC EXPRESS will provide the following:

PACKAGE #1: Mini Express 1 to 3 hour’s /  DJ , your choice of music

         PACKAGE #2: Basic 4 hour’s /  DJ , your choice of music and a light show

            PACKAGE #3: Extended 5 hour’s / DJ , your choice of music and a light show

                                       PACKAGE #4: Super Extended 6 hour’s or More / DJ , your choice of music and a light show

              PACKAGE #5:  Karaoke Sing-a-long.a professional Karaoke system includes,

  cordless mic, song books, monitors, Karaoke stand, speakers, mixer and

2 Karaoke players to keep your party singing non stop with no dead air.

Motivational Dancers with a DJ package for an additional charge.

Karaoke available with any DJ package for an additional charge.

                                                has agreed with this contract to pay MUSIC EXPRESS DJ SERVICE a deposit of $100.00 to confirm entertainment                 for the __________day of                           ,    with the remaining balance of $____________ due payable day of the event.

**NOTE: Deposits are refundable within a 5 day notice of cancellation.  Incase of inability to perform, due to certain circumstances beyond    our control and This contract is void at the options of  both parties.  Please return this contract and a $100.00 deposit as soon as possible to                   secure your date.  Instructions listed below are agreed upon by__________________________  and  MUSIC EXPRESS.
